1.The application from the prospectus will be issued in the month of July from the office of the Principal Jeevan Jyoti Paramedical Institute, Jatepur Gorakhpur on the Payment of Rs. 500.
2. Application Should be filled by the Candidate in her own handwriting.
3. Application Form duly filled in together with photocopies of all necessary documents should reach the office of the Principal before ………………………..of the respective year.
4. Originals of the attested copies are to be produced at the time of interview.
5. Incomplete application will not be entertained.
6. The following documents should be attached with the application form.
a) True copies of the mark sheet and certificates of high school intermediate or equivalent examination.
b) School leaving certificate showing the date of birth.
c) Character certificate from the principal of the college last attended.
d) Fitness certificate from a registered medical officer.
Interview and Selection
1) Provisionally selected candidates shall be intimated by the Principal of school of Paramedical Institute
2) Selection and admission will be based on merits of qualify written test and personal interview.
3) Preferences will be given to candidates from U.P. State.
4) The decision of the admission committee of the institution will be final and binding.
5) Selected candidates will have to undergo a complete physical Examination conducted by the institution and only those fit will be selected for the courses.
